During these consultations we will:
- Provide a medical assessment to determine the appropriate form of transportation
- Review and determine insurance benefits
- Develop a patient transport plan specifically tailored for each individual
Some of the information that we require from you when booking a flight are:
- History and Physical
- Face Sheet/Patient Demographics
- Insurance Information (to determine benefits for the patient)
- Medication Administration Records (MARS)
